WAIVER OF LIABILITY, ASSUMPTION OF RISK AND RELEASE AGREEMENT READY CAREFULLY

1.  I voluntarily agree to sign this waiver and assumption of risks, because I fully understand that Deborah Picou Markowski, who is going to perform a Quantum Healing Hypnosis Technique, Emotion Code, Body Code, Akashic Reading Consultation, Healing Energy Transmission &/or Chakra Balancing Session is not doctor, nor has a degree in Psychiatry, does not diagnose nor treat any type of physical or mental disorder, nor any type of condition. I understand that this hypnosis session is exclusively for entertaining purposes only. Moreover, I understand that any given or implied suggestion is not to be construed as medical, psychiatric or physiological advice.

2. I understand that it is my responsibility to consult with a physician prior to the session. I represent and warrant that I am physically able to fully participate in a Quantum Healing Hypnosis Technique, Emotion Code, Body Code, Akashic Reading Consultation, Healing Energy Transmission & Chakra Balancing session.

3. Assumption of Risks: In consideration of being permitted to participate in a  session, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in any such Quantum Healing Hypnosis Technique, Emotion Code, Body Code, Akashic Reading Consultation, Healing Energy Transmission & Chakra Balancing session.

4. Waiver: In further consideration of being permitted to participate in a Quantum Healing Hypnosis Technique, Emotion Code, Body Code, Akashic Reading Consultation, Healing Energy Transmission & Chakra Balancing session, I knowingly, voluntarily and expressly waive, release and discharge, any claim that I, for myself, my heirs, personal representatives or assigns may have against Quantum Healing Possibilities and/or Deborah Picou Markowski or their respective officers, managers, directors, members, employees and agents for injury or damages that I may sustain as a result of participating in a Quantum Healing Hypnosis Technique, Emotion Code, Body Code, Akashic Reading Consultation, Healing Energy Transmission & Chakra Balancing session.

5. Indemnification and Hold Harmless: I agree to hold harmless and indemnify Deborah Picou Markowski and/or Quantum Healing Possibilities from all claims, actions, suits, procedures, costs, expenses, damages and liability, including attorneys fees incurred, and to reimburse them for any such or further expense incurred Quantum Healing Possibilities and/or Deborah Picou Markowski in investigating and defending a claim or suit brought by me, my heirs, personal representatives or assigns.

6. Severability: Finally, I agree that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the laws of the State of Florida, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

7. I am willing to be guided through relaxation, visual imagery and/or hypnosis. I am aware these modalities are spiritual-based and non-medical in nature and it is my responsibility to consult my regular doctor about any changes in my condition or changes in my medication. I understand the above modalities are not substitutes for regular medical care and I have been advised to consult my regular medical doctor or health-care practitioner for treatment of any old, new or existing medical conditions. I understand that being hypnotized is not being asleep. During a deep hypnotic trance, you can open your eyes, speak, laugh, walk and you may be aware of everything that happens around you. You can even open your eyes and think it is not working and are not hypnotized. But when you allow those feelings or thoughts that come to your mind to flow freely, you will relax and remember forgotten events in this life or past life. I understand that change is my own and complete responsibility. I understand all healing is self-healing and that Deborah Picou Markowski is only a facilitator/ conduit in the process of helping me to solve my own problem(s). It is my responsibility to be open and honest, provide accurate feedback and be forthcoming with details and information that may help me achieve my outcomes. I understand that our session will be digitally recorded for my later use. I also understand that in these types of metaphysical sessions, the energy in the room can affect the equipment and recording resulting in static or blank recordings. I agree that no compensation will be paid for any products or revenues or any other value derived from these recordings or any resulting products. I waive all rights from the use of such recordings. I do not ask for, nor expect, any compensation from any of the recordings taken during the hypnosis session.

8. Special Use of Information: I understand that my name and personal information will be kept completely confidential. I understand that often in Hypnosis Sessions, universal information is provided through the client to benefit all of humanity. I agree to allow Deborah Picou Markowski and Quantum Healing Possibilities to share this information and any accompanying story on Quantum Healing Possibilities’s Website, Youtube and Social Media as long as my name and all personal and relevant details are omitted or changed.
